If you live within the practice area and wish to register with us you will need to bring your medical card along with you to complete the registration form. Additional proofs of identity are required, one of which should be photographic (e.g. passport or new style driving licence), and a recent utility bill displaying your name and address.

It is necessary for all new patients wishing to register with the practice to book an appointment for a health screening examination (For people aged 16 years and over) with a practice nurse. This is to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.

Changes to Personal Details

Please advise reception if you change your name, address or telephone number. It would be a great help if you would also ensure that we have your telephone number and your postcode.
