REGISTER AS A NEW PATIENT?
If you live within the practice area and wish to register with us you will need to bring your medical card along with you to complete the registration form. Additional forms of proof of indentity will be required, one of which should be photographic (eg passport or new style driving licence), and a recent utility bill displaying your name and address.
It is necessary for all new patients wishing to register with the practice to book an appointment for a health screening examination with a practice nurse.
